Critically analyse and evaluate the health needs assessment for the care of individuals, families and groups and demonstrate the ability to plan care, using an evidence base.

Critically appraise the assessment, diagnosis, treatment & care of patients with long term conditions, in accordance with national and local protocols and policies.

Instigate and facilitate strategies and evidence based interventions, which can be used to promote the health of individuals, carers, families and groups.

Demonstrate a creative approach with a range of communication methods, networks and working relationships within the practice setting, that promote and enhance the learning process.
