Aims

To facilitate the development of a safe and effective Clinical Endoscopist (CE), by equipping the student with the knowledge, skills, competences to perform Lower Gastrointestinal (GI) Endoscopy and Polypectomy level 1, incorporating the professional attributes that underpin an advanced practice role.

Learning Outcomes

1.
Demonstrate a critical understanding of the broadened level of responsibility and autonomy of the advanced practice role and the professional scope of practice set within legal, ethical, professional and organisational governance.
2.
Critically examine the principles and practice of the Clinical Endoscopist role and critically apply knowledge of the key concepts involved in advanced clinical reasoning and decision making.
3.
Critically appraise the evidence base underpinning the Clinical Endoscopist role in relation to consent, effective patient report writing, legal, regulatory and ethical aspects of advanced practice.
4.
Obtain relevant information by history taking, physical assessment and examination of a patient's clinical records and synthesise this information to guide interpretation of endoscopic findings.
5.
Acquire and synthesise all necessary evidence relevant to effectively manage patients with individual or complex comorbidities when presenting for Lower Gastrointestinal (GI) Endoscopy and Polypectomy level 1.
6.
Interpret endoscopic findings based on knowledge of the anatomy, physiology and pathophysiology of the lower gastrointestinal tract and synthesise this information to formulate a differential diagnosis, recommend further investigations and develop an evidence-based management plan.
7.
Demonstrate competence in endoscopic skills, both technical and non-technical, as defined by the Joint Advisory Group (JAG) in relation to diagnostic Lower Gastrointestinal Tract Endoscopy and Polypectomy level 1

Module Content

Outline Syllabus:
• The principles and practice of Clinical Endoscopy. • Anatomy & physiology of the Lower G.I. tract. • Pharmacology for lower GI Endoscopy and safe sedation practice. • Medico-legal issues and ethical implications including consent/ communication skills. • Physical assessment, history taking and clinical examination. • Indications for lower GI Endoscopy, principals of pre assessment, patient selection, risk management, co morbidity issues and preparation for endoscopic procedures. • Human factors, non-technical and technical skills of endoscopy and equipment / simulator training, introduction to Colonoscopy and Polypectomy level 1. • Pathophysiology and histopathology of the Lower G.I. tract. • Therapeutic endoscopy practice – biopsy, pathology specimens and Polypectomy. • The Anus and Rectum master class. • The causes of rectal bleeding and their management. • Colonic Polyps and Tumours. • An introduction to Inflammatory Bowel Diseases (IBD). • Understanding Diverticular Disease and its complications. • A diagnostic approach to Diarrhoea. • Understanding functional disorders of the lower GI tract. • Emergency management and complications during lower GI Endoscopy. • Health promotion and management for the lower GI Endoscopy patient. • Professional issues and advanced practice. • Role development, continuing education and career pathways for Clinical Endoscopy. • Service development, evaluation and business planning. • Relationships within the MDT. • Links with JAG / JETS.
Module Overview:
This module will facilitate the development of a safe and effective Clinical Endoscopist (CE), by equipping you with the knowledge, skills, competences to perform Lower Gastrointestinal (GI) Endoscopy and Polypectomy level 1, incorporating the professional attributes that underpin an advanced practice role.
Additional Information:
This module is specifically designed to meet the requirements set by Health Education England (HEE) for a national Clinical Endoscopist (CE) Training Programme. The aim of which is to have a clinically based (7 months), academically focussed learning programme to meet the outcomes of the JAG Endoscopy Training Scheme (JETS) and the Health Education England Clinical Endoscopist Competencies. The overall aim of the module is to adequately prepare practitioners in this new and complex role and to maintain patient safety. Teaching will take place on site or via the Virtual Learning Environment, the students will return to their practice settings to reflect upon and develop expertise within their own settings supported by a local consultant Medical Clinical Supervisor and an Advanced Practice Mentor. To meet the learning outcomes students will be offered blended learning opportunities based upon the following elements:- 1. Clinical / practical skills training 2. Completion of the CE competency portfolio 3. Academic element 4. Completion of SLATE e-learning package 5. Basic Skills course 6. Clinical supervision and personal development The course will be informed by the Quality Assurance Agency for Higher Education Benchmark Statement for Health and Social Care Programmes and students on the course will be expected to adhere to their relevant Professional Code of Practice. Criteria for Admission: The criteria for potential students admission to the module requires the student must demonstrate the following: 1. Registered health care professional 2. Substantive post registration experience 3. Working with patients with Gastrointestinal Disease or Cancer. 4. The applicant who is studying towards/ have a Master’s Degree level education but no previous endoscopic skill experience will be considered by the module leader, to assess the applicants capacity to undertake lower GI Endoscopy and academic study at level 7. 5. Students with no Master's Degree level education but have J.A.G. accreditation in Oesophago-Gastro-Duodenoscopy (OGD) and are able to demonstrate competency in existing skill via performance data will assessed by the module leader to determine the applicant’s capability to study at level 7. 6. Participation in audit/QA/research and changing practice 7. Fully supported by senior managers within their organisation, demonstrated with a bespoke business and training plan. The module learning outcomes and relevant subject benchmark statements are in line with the QAA framework for higher education qualifications in England, Wales and Northern Ireland. The module is aligned to the Level 7 descriptors for a Level 7 Higher Education Qualification. The module is part time, delivered over a seven month period, twice per year. After the seven month period the student will be required to have successfully completed all required components of the module. In line with Health Education England professional requirement to maintain patient safety, no referral will be offered on this CPD. The final award is a Continuing Professional Development Introduction to Lower GI Endoscopy part 1 The students have access to Canvas and the University's range of other electronic support such as access to the electronic library facilities. The Programme Leader will provide academic student support via a range of mechanisms including, phone contact, e-mail; Classroom Contact (Lectures; Seminars; Tutorials); Canvas and Personal Tutorials for Pastoral Support as appropriate. A module guide will also provided, which guides students to the wider range of support available. Continuous student support and clinical review will be provided by the consultant Medical Clinical Supervisor and an Advanced Practice Mentor. These two individuals will be responsible for arranging J.A.G sign off as competent on JETS and on the CE portfolio.
