Aims

This module aims to develop students’ ability to optimise medicines for individual patients (including those with multi-morbidity) by updating their knowledge of clinical therapeutics and skills in its application in real life practice.

Module Content

Outline Syllabus:
  • Policies and guidance relating to medicines optimisation
  • Consultation skills
  • Shared-decision making
  • Medicines adherence
  • Behavioural change and motivational interviewing
  • Multi-morbidity and frailty
  • Polypharmacy, over-prescribing and approaches to de-prescribing
  • Safe use of medicines
  • Medicines reconciliation and transfer of care
  • Clinical reasoning and decision making
  • Antimicrobial stewardship
  • High risk drugs
  • A selection of long-term conditions will be used to illustrate the aforementioend concepts. The following list is not exhaustive and may change based on emerging evidence and national priorities:
    • Respiratory disease
    • Diabetes
    • Cardiovascular disease  
    • Chronic kidney disease
    • Chronic pain and musculoskeletal disorders
    • Mental health
    • Living with cancer
Module Overview:
This module is for the PGCert, PGDip and MSc. This module aims to develop students’ ability to optimise medicines for individual patients (including those with multi-morbidity) by updating their knowledge of clinical therapeutics and skills in its application in real-life practice.
Additional Information:
The majority of this module involves work-based training under the guidance of a Practice-based tutor (PBT), following a prescribed schedule. The student and PBT are also supported by the Clinical Liaison Tutor team.
