Aims

The aim of this module is to develop a critical awareness of the knowledge fundamental to the contemporary role of the district nurse.

Learning Outcomes

1.
Critically analyse the health needs assessment for the care of individuals, families and groups and demonstrate the ability to plan care, using an evidence base.
2.
Critically reflect on the strategies and evidence based interventions, which can be used to promote the health of individuals, carers, families and groups.
3.
Critically analyse the assessment, diagnosis, treatment & care of patients, with long term conditions, in accordance with national and local protocols and policies.
4.
Critically analyse a variety of communication methods, networks and working relationships within the practice setting, that promote and enhance the learning process.

Module Content

Outline Syllabus:Professional values and behaviours - 6Cs. Professional responsibility, accountability confidentiality, legislation and record keeping. Effective caseload management and referral - complex care co-ordination, assessment, diagnosis, planning, implementation of planned care, evaluation of care plans. Partnership working and integrated care using evidence based approaches. Assessing the needs of older people and carers. Mental Health and social isolation. End of life Care. Long-term conditions management. Valuing diversity. Individualised innovative care planning - supporting patients to self care, promoting independence, empowerment and partnership approaches with the patient as a partner. Technology assistance to support the health and care of patients, admission avoidance, integrated care methods, clinical nursing care processes and protocols. Exploring health needs and health promotion. DH, PHE and NHS policy. Health & Safety. Risk Management. Audit. Principles of learning & facilitation of learning for the development of staff and learners.
