Aims
To provide the student with the necessary knowledge and insight to ensure a coordinated approach to the management of patients/clients with a long term condition.
Learning Outcomes
1.
Demonstrate critical knowledge of the physical, psychological behavioural and socio economic impact of living well with a long term condition and multi-morbidity with specific focus on common clusters of conditions
2.
Critically discuss the national and international policy context for people with long term conditions whilst tailoring local services and interventions to meet the personal outcomes of the individual
3.
Critically explore the barriers to multi-disciplinary, inter-agency and partnership working in the management of long-term conditions, and synthesise possible solutions
Module Content
Outline Syllabus:Aetiology and diagnosis of long term conditions, pathophysiology of long term conditions, complications related to long term conditions, multi-morbidity, pharmacology related to long term conditions, prevention of long term conditions.
Rehabilitation, NICE guidance, Self care, promoting independence, empowerment, health literacy and the Expert Patient.
End stage complications, Palliative care, Partnership working, shared care arrangements, admission avoidance, legalities involved in Long term Conditions, psychological care, diversity.
Additional Information:Engagement in this module will focus on the approaches to providing nursing care for patients with long term conditions whilst advancing your understanding of person-centred approaches to improving services for people with multiple conditions globally, as identified in national and international policy. The topic must be agreed with the tutor beforehand, and must
be relevant to nursing.
