Aims
To facilitate the development of a safe and effective Clinical Endoscopist (CE), by equipping the student with the knowledge, skills, competences to perform Lower Gastrointestinal (GI) Endoscopy and Polypectomy to a level appropriate for Colonoscopy practice, incorporating the professional attributes that underpin an advanced practice role.
Learning Outcomes
1.
Demonstrate a critical understanding of the broadened level of responsibility and autonomy of the advanced practice role and the professional scope of practice set within legal, ethical, professional and organisational governance.
2.
Critically examine the principles and practice of the Clinical Endoscopist role and critically apply knowledge of the key concepts involved in advanced clinical reasoning and decision making.
3.
Critically appraise the evidence base underpinning the Clinical Endoscopist role in relation to consent, effective patient report writing, legal, regulatory and ethical aspects of advanced practice.
4.
Obtain relevant information by history taking, physical assessment and examination of a patient's clinical records and synthesise this information to guide interpretation of endoscopic findings.
5.
Acquire and synthesise all necessary evidence relevant to practice to effectively manage patients with individual or complex comorbidities when presenting for Lower Gastrointestinal (GI) Colonoscopy and Polypectomy level 2.
6.
Interpret endoscopic findings based on knowledge of the anatomy, physiology and pathophysiology of the lower gastrointestinal tract and synthesise this information to formulate a differential diagnosis, recommend further investigations and develop an evidence-based management plan.
7.
Demonstrate competence in endoscopic skills, both technical and non-technical, as defined by the Joint Advisory Group (JAG) in relation to diagnostic Lower Gastrointestinal Tract Endoscopy (Colonoscopy) and Polypectomy level 2
Module Content
Outline Syllabus:
- The principles and practice of Clinical Endoscopy.
- Anatomy & physiology of the Lower G.I. tract.
- Bowel Cancer and Polyps: epidemiology. aetiology, pathology, investigations, diagnosis, treatments and outcomes.
- Pharmacology for Colonoscopy and safe sedation practice.
- Medico-legal issues and ethical implications including consent/ communication skills.
- Physical assessment, history taking and clinical examination.
- Indications for Colonoscopy, principals of pre assessment, patient selection, risk management, co morbidity issues and preparation for Colonoscopy.
- The assessment, differential diagnosis and investigation of diarrhoea; including the use of Faecal Calprotectin and Faecal Immunological testing for Haemoglobin.
- Bowel preparation: optimisation, contraindications and complications.
- Inflammatory Bowel Diseases (IBD): differential diagnosis, treatment principles and surveillance.
- The presentation and management of Infective and Inflammatory Colorectal disorders (other than Idiopathic Inflammatory Bowel Disease).
- Understands the role of Colonoscopy in the investigation of Anaemia.
- Radiological investigations in the assessment of lower GI disease.
- An introduction to the genetics of Colorectal Cancer.
- Understanding quality assurance in Colonoscopy.
- Detection and excision of flat Polyps and safe level 2 Polypectomy.
- Colonoscopic surveillance guidelines for Cancer, Adenomas and Serrated Lesions.
- Colonoscopic screening for individuals with a family history of Colorectal Cancer or predisposing comorbidity.
- The principles and practice of National Bowel Cancer Screening Programmes (BCSP).
- Human factors, non-technical and technical skills of endoscopy and equipment / simulator training for Colonoscopy and Polypectomy.
- Pathophysiology and histopathology of the Lower G.I. tract.
- Therapeutic Colonoscopy practice - biopsy and Polypectomy.
- Emergency management and complications during Colonoscopy
- Health promotion and management for the Colonoscopy patient.
- Professional issues and advanced practice.
- Role development, continuing education and career pathways for Clinical Endoscopy.
- Service development, evaluation and business planning.
- Relationships within the MDT.

Module Overview:
The Clinical Endoscopist module cater to a national footprint and adopts an innovative, blended approach to learning. The module contents are delivered by experts in the field of Endoscopy or Advancing Practice via an interactive virtual classroom. This module places an emphasis on robust clinical leadership, advanced communication skills, in-depth anatomy, physiology, pathophysiology, image interpretation, autonomous decision-making, diagnosis, evidence base patient management. The curriculum and delivery design incorporates technical and non-technical Endoscopy skills within a simulated learning environment. This enabled LJMU to provide a comprehensive educational programme to meet the students needs, ensure J.A.G. standards are achieved, is cost effective and contributes to enhanced patient safety.

This module is specifically designed to meet the requirements set by Health Education England (HEE) for a national Clinical Endoscopist (CE) training programme. The aim of which is to have a clinically based (9 months), academically focussed programme to meet the outcomes of the JAG Endoscopy Training Scheme (JETS) and the Health Education England Clinical Endoscopist Competencies. The overall aim of the module is to adequately prepare practitioners in this new and complex role and to maintain patient safety.
Teaching will take place on site or via the Virtual Learning Environment, the students will return to their practice settings to reflect upon and develop expertise within their own settings supported by a local consultant Medical Clinical Supervisor and an Advanced Practice Mentor.
To meet the learning outcomes students will be offered blended learning opportunities based upon the following elements: -
The course will be informed by the Quality Assurance Agency for Higher Education Benchmark Statement for Health and Social Care Programmes and students on the course will be expected to adhere to their relevant Professional Code of Practice
In line with Health Education England professional requirement to maintain patient safety, no referral will be offered on this CPD.
