Aims
This module aims to develop the clinical knowledge and skills required by hospital pharmacists for managing medicines in patients that are admitted to hospital
Learning Outcomes
1.
Clinically assess patients’ medicines-related needs when they are admitted to hospital
2.
Choose, or make recommendations on, the most appropriate treatment for acute medical and surgical problems by applying their knowledge of pharmacology, pharmacokinetics, guidelines and published evidence
3.
Assess the impact of acute illnesses on patients’ medicines-related needs (including consideration of drug administration issues and altered drug handling)
4.
Formulate robust and realistic monitoring plans for patients’ medicines, acknowledging the responsibilities of different healthcare professionals
Module Content
Outline Syllabus:• Altered drug handling in acute illness
• Drug administration challenges
• Antimicrobial stewardship
• Management of common reasons for hospital admission / problems encountered during hospital admissions – the emphasis specific areas may change each year depending on emerging evidence and changes to national guidelines. The priority areas will normally be expected to be:
o Common infections – e.g. pneumonias urinary tract infections, skin and soft tissue infections
o Healthcare-acquired infections
o Sepsis
o Venous thromboembolism
o Exacerbations of asthma and COPD
o Acute coronary syndromes
o Arrhythmias
o Acute heart failure
o Diabetic emergencies
o Acute kidney injury
o Alcohol withdrawal
o Acute confusion / delirium
o Upper GI bleeds
o Decompensated liver disease
o Inflammatory bowel disease (IBD) flares
• Medicines optimisation for patients undergoing surgery
• Management of patients on high dependency and critical care units

Additional Information:The majority of this module involves work-based training under the guidance of a Practice-based tutor (PBT), following a prescribed schedule. The student and PBT are also supported by the Clinical Liaison Tutor team.
