Aims

To provide the district nurse with the skills and knowledge required to perform clinical assessments of patients in the community setting. To provide the district nurse with skills in interpreting relevant diagnostic tests which may inform treatment plans, treatment modification or a suitable referral.

Learning Outcomes

1.
Critically evaluate the range of national and local drivers that effect multi-disciplinary, interagency and partnership working in the management of acute and long-term conditions and examine social, cultural, contextual and pathophysiological dimensions of disease in primary care.
2.
Apply underpinning knowledge and demonstrate competence in the comprehensive clinical examination and assessment of patients with acute and long term conditions.
3.
Demonstrate a systematic understanding of a clinical history and examination to make an appropriate referral to other members of the healthcare team.
4.
Demonstrate a comprehensive ability to integrate knowledge of homeostatic mechanisms and disordered physiology in the assessment of patients in the community setting.

Module Content

Outline Syllabus:Foundations of history taking. Clinical assessment of the following body systems: Cardiovascular, Respiratory, Abdominal, Neurological. Interpretation of haematological investigations
Module Overview:
During this module, you will gain the skills and knowledge required to perform clinical assessments of patients in the community setting. It will also help you develop skills in interpreting relevant diagnostic tests which may inform treatment plans, treatment modification or a suitable referral.
Additional Information:On completion of this module students will be able to apply knowledge of disease process, history taking, and clinical examination and arrive upon a clinical impression leading to the development of a treatment plan and appropriate referral. The OSCE is marked as pass/refer.
